Collected information by Healthy Progress. The following is information that is required; first and last name, degree, specialty, city, state, zip code, email. The purpose of the information is for statistical analysis only and available to Healthy Progress staff only. The personal information will not be sold or given away at any time to a third party.

Subscription Services. Each proposed subscription will require its own Service Agreement which will govern use & terms of the service.

Patient's Information. Healthy Progress temporarily collects the name of the patient. However, the name is not saved in the data base and is lost once the exercise prescription is completed.

Data Collected. We do not collect Non-Personal Information from Healthyprogress.com site users. However, with User authorization Healthy Progress would like the opportunity to collect information to improve services. This information will not be shared.

Disclosure of your information. Healthy Progress will not disclose any Personal Identifiable or Personal Health Information it gathers from you on our website. We may only release information for the purpose of compliance with valid legal requirements such as is warranted by local or federal government agency, search warrant or court order. Should such an order be presented, we will make efforts to notify you unless doing so would violate the law. Operation and maintenance contractors will have limited access to your Personal Identifiable Information such as email addresses in order to provide technical services and operation maintenance of Healthyprogress.com.
